Please join us for a FREE workshop on Advance Directives, providing an opportunity to define your end-of-life wishes regarding treatment guidelines and to appoint a Health Care Agent. We will discuss and help you complete any or all of three documents: Health Care Proxy, Living Will, and MOLST
